British Airways Flight 5390- Blow Out

Synopsis

British Airways Flight 5390 was scheduled to operate a route from Birmingham in England to Malaga in Spain on 10th June, 1990. BAC – One Eleven was the type of aircraft that was used in this flight. The windscreen in front of the Captain’s seat suddenly shot out to the sky in a second, half of the Captain’s body got blew outside the windscreen immediately at 17,300 feet and lying against the left hand side of the side window of the cockpit. With the team work from the co- pilot and the cabin crews, the co- pilot could stay claim and stable to make logical decision during the event, he had managed to land the aircraft at Southampton Airport with no life loss and the Captain amazingly survived after being physically exposed outside the cockpit for approximately 20 minutes. The Captain lost his consciousness due to thin air; his body was frozen in 500mph slipstream.

The cause of the windscreen blew out was because the sizes of the bolts for the windscreen were wrongly replaced the night before the flight. the replacements of the bolts on the windscreen could not support the decompression of the cabin. The engineer who embedded these bolts ignored the procedures and professional advice, fitting the wrong size of bolt to the windscreen while he was working under pressure, he made the decision due to the strong psychological believe he had in his own knowledge and expertises. This was the first accident report which involved Human Factors in Maintenance and Engineering department of the aviation system.
