Birgenair Flight 301- Underlay error

Synopsis

On 6th February, 1996, Birgenair Flight 301, a Boeing 7357- 225 jet crashed not long after it took off from the Gregiro Luperon International Airport Dominican Republic to the destination Germany. The 2 out of 3 Air speed Indicators were not working causing confusion from the Captain’s decision making of the flight. The plane crashed into the Caribbean Sea 6 minutes after taking off from the airport causing a fatality of 13 crews and 176 passengers.

Active Error:
The pilots failed to recognize the activation of the stick shaker as a warning of causing the stall as well as the crews did not follow the procedures from recovery from the onset of loss of control.

Latent Error:
The investigators suspect that the blockage of the pilot tubes were probably caused by specie called Black and yellow mud daubers were nesting in the tubes since the aircraft had not flown for 25 days. Therefore the air speed indicator did not function properly.
